If you would like addtional information regarding private lessons, please fill out this online form. J D will contact you personally. You will receive an email detailing all of the information about private lessons in the home with ThomaStudios. Please read that email carefully and completely. Rest assured that all the information you provide here will be used only for internal ThomaStudios purposes. It will NEVER be shared, sold, or made available to anyone else, EVER.

A few things to keep in mind before you fill out this form:

All items marked with an asterisk* are required for this form to be processed properly.
Currently, private lessons in your home are limited to West Linn and Lake Oswego only.

Lessons in the home are scheduled weekly. Bi-weekly (every other week) or less frequent are NOT available in the home. See 'About Scheduling Lessons' on this page for more information.
Filling out this form is NOT a binding contract to sign up for lessons. But, if you do sign on, please take a moment to read and agree the the cancelation policy detailed here. You will be making a tacit agreement to this policy should you start lessons with JD.
